- Complete forms with a Doctor include a photo (where needed) and review date
- Complete the Medication Agreement if required
- Complete the Risk Minimisation Plan
- Scan and upload documents to Fully Booked
- A review 12 month review date will be added to your uploaded documents in Fully Booked.
- An OSHC educator may contact you about this documentation
(Fully booked will send you a reminder to update information closer to the expiry date)

DEPARTMENT OF EDUCATION
Health Care Plan is to be completed for every child who has a disability or health condition. Please include your child’s photo on these forms. Please upload the documents and expiry date onto FullyBooked.
REDWOOD PARK OSHC
If your child requires medication while at OSHC please complete with a Doctor. All medications require prescription, child’s name, Dr’s full name and expiry date. Please upload the documents and expiry date onto FullyBooked.
